Case Managers assist families to locate service providers and funding.
Who For:
Children 5 years old through adults with intellectual and/or developmental disabilities. Sheltered Living serves individuals with over 100 diagnoses.
What will a Sheltered Living case manager do for my family?
Your Sheltered Living Case Manager will:
1. Assist you to determine your family’s eligibility for state and federal programs.
2. Help you develop the Person Centered Support Plan.
3. Assist you to prepare SSI, Medicaid, Homestead and LIEAP paperwork.
4. Sheltered Living case managers will make both home and work visitations.
5. If your child is still in school, the case manager will assist in advocating for services.
How do I start?
You may call the Shawnee County Community Developmental Disabilities Organization at 785-232-5083. They will assist you to determine eligibility and give you a list of providers in Shawnee County. However, we would be glad to help contact and set up appointments.
Did you know?
When your child is 5 years old you may put him / her on the waiting list. At present there are over 4,000 people on the waiting list for services in Kansas.
You have the right to be involved in transitioning your child between schools. This includes:
1. Stating your preference in schools and classroom environments.
2. Requesting additional services specific to your child’s needs.
You may be eligible for a Supplemental Support Grant.
Your child may be eligible for SSI (Supplemental Security Income).
Your child may be eligible for Medicaid.
Your child may be eligible for Social Security.
An adult receiving Medicaid and / or SSI must spend-down his bank account to $2,000 or less.
If you want to be involved in decision making for your child when he turns 18 (becomes an adult) then you must apply to the court before that time. Options include conservatorship, guardianship, power of attorney and legal payee status.
